Undergraduate Late Drop and Semester Withdrawal

Instructions:
1. Tell Us What Happened –
Attach a typed statement that explains your situation. Be specific and honest. Include:
- A timeline of what happened (with dates and names, if relevant).
- How the situation affected your ability to do well in your classes.
- Why you weren’t able to drop the course(s) before the official deadline.
- Why you're only dropping one class, some classes, or all classes (whichever applies).
- How this drop or withdrawal will help you stay on track and succeed going forward.
2. Show Proof – Upload Supporting Documents
Attach documents that support your situation. They must include your name, dates, and a brief description. Submit only the relevant pages. You may redact unrelated or sensitive information.

- Medical Issues: Completed Healthcare Provider Form [pdf] or note from a licensed provider.
- Legal Issues / Divorce: Court order, police report, eviction notice.
- Accident / Natural Disaster: Insurance claim or official report.
- University Error: Email or letter from ÎÞÓǶÌÊÓÆµ staff/faculty (on letterhead or from an @sjsu.edu address).
- Military Duty: Official orders or letter from a Commanding Officer.
- Caregiver Responsibilities: Healthcare provider letter, legal document, or birth certificate.
- Personal Hardship: Support letter from your advisor or instructor (they can also help guide what to include).
3. Choose the Right Form – Petition Links
Fill out the correct online form through DocuSign (Available 8:00 am, June 5, 2025 to 5:00 pm August 08, 2025):
- – Use if you're dropping one or two courses.
- – Use if you're dropping all your courses for the term.
Note: requests related to the non-receipt of financial aid during the summer/winter sessions will be denied. See FAQ's for details.
