Follow the steps below to access e-forms.  E-forms are only available to F-1/J-1 students and scholars. 

Option 1

1. Search: 
2. Click 鈥淟og in鈥� with your 无忧短视频 credentials
3. Click on the category to search for e-forms.

Option 2 

(If you have issues with logging in one.sjsu.edu or DUO authentication, do not use this option). 

1. Log in to  using your credentials
2. Search for 鈥渋Spartan鈥� app
3. Click on the category to search for e-forms.
